How Do I apply for Assistance?

We are glad you have visited our website today, and we hope that we can be of assistance. We must screen all applicants to verify a legitimate need of funds. Certain documentation is needed to complete your request. Please find a list below of what information is needed.

All information is reviewed confidentially and will not be shared with anyone outside of the necessary communications with foundation board members, medical and financial institutions, and those needed to approved the application.

Please send a written request explaining your situation and the specific need that you have.

In your letter please include:

• Your name (Patient's Name)
  Patient's Address and phone number
  Your name if you are requesting on the behalf the patient
  Your address and phone number
• Please also include with your mailing:
• Documentation from patient's physician stating the patient's condition and medical need.
  Documentation from the transplant facility expected to perform the transplant.
  Documentation showing the patient listed on the transplant list.
• Copies of the most recent two years' tax returns.
  Copies of the most recent two years' bank statements.
• Documentation of insurance coverage or lack of insurance coverage.

All requests will be reviewed by our board members and other officials. We regret that not all requests will be granted. However, those with the most urgent and greatest need will be given diligent consideration.Our hope is that you and your family are blessed, whatever your situation, and that you are able to acquire the necessary medical attention required to meet your need.
